Major lenders have sent these type of letters out to tens of thousands of customers.  In some cases there are blanket freezes going on for certain parts of the country.

Given the drop in home prices and the tightening credit crunch many lenders have been reviewing their portfolios (read: your home loan).  And when they run automated home valuations they see that they’re on the hook for potential losses.
Can they really do this?

Yes they can.  In all equity lines agreements there is a standard clause that gives the Denver corporate housing lender to cancel or freeze the line in case of a drastic reduction of home value.  Since the line is also the 2nd lien on your house the lender usually doesn’t get much if the house goes to foreclosure.

So what is there to do?  If you think that the Automated Valuation that the lender used could be really wrong and your house hasn’t declined that much you can pay for an appraisal and send it in as proof.  This doesn’t guarantee that they will unfreeze it but it can be worth a shot if you like your current line and the terms.  Keep in mind an appraisal is around $300 though.

Usually the best option is to shop around for a new lender.  Keep in mind though that the lending criteria has tightened quite a bit.  Many major lenders don’t write home equity with a total loan-t0-value ratio of either 80 or 85% of the home’s value.  Basically 80-85% is the new 90-95% who used to be the criteria a year or two ago.

Your best bet?  Not the big nationals right now.  It’s going to be your local small bank or credit union.  Some credit unions are still writing home equity lines up to 95% of the home value (minus balance of the first mortgage).

Knowing what to change about yourself and finding enough motivation is easy compared to remembering and sticking with what you have to do in order to change, especially when it comes to weight loss. Everyday your mind and body is trying to get back to doing what it was used to doing, because that’s what was comfortable. Your body instinctively reaches for candy bars, hot dogs, or whatever food got you over weight in the first place, making it easy to bypass the new health ideals you’re trying to promote. The key is to condition the body and remind yourself of your motives with whatever you can so that the new lifestyle becomes the new natural and instinctive thing to do.

I always wondered how some people get so good at playing sports or a musical instrument. It amazes me how well some guys I know can play the piano. They play so well because it is their passion. They think about playing music all the time, even when they don’t really have anything else to think about. Learning such a passion about something is possible.

With my goals, I like to make rules for myself. For example, I wanted to tone down the amount of time I spent on video games. I didn’t like how much they controlled my life. It’s never enough for me to say, “OK, I’m not going to play video games anymore” and then be done. I need something else to do or else I fall back on my old habit. So I told myself that instead of playing video games I would do something from a list I wrote out of things to make me more productive; they were simple things like writing, reading and playing my ukulele.

Every morning I would review the list and get my goal of not playing games fresh in my head. That helped me set the tone for the rest of the day. Those review sessions every morning helped me refocus and remember why I wanted to stop playing video games in the first place. It helped me develop a passion for what I saw a more productive life. I never quit playing video games completely, but I got to a comfortable balance.

The same principle of reminding yourself of your motivation applies to losing weight. Set up a system that will constantly remind you of your goals and stick to it. That way the change you want is long lasting.

Most people use tanning beds wearing as little as possible, and exposing too much surface area of the skin can quickly result in excessive absorption of ultraviolet light.

So as you can see, a balance has to be struck between exposing your skin to as much light as possible, while protecting it from damage. WEDNESDAY, Nov. 3 (HealthDay News) — People with a common genetic variant that’s associated with autism have a “disconnect” between their frontal lobe and other areas of the brain important for language, brain scans show.

The disconnect may help explain some of the language and communication difficulties that are characteristic of autism, researchers report in the Nov. 3 issue of Science Translational Medicine.

About one-third of all people carry the variant of the CNTNAP2 gene that is associated with a heightened risk of autism, as well as attention-deficit/hyperactivity disorder, Tourette syndrome, schizophrenia and other language difficulties.

In the study, researchers performed functional MRI brain scans — which measure blood flow in the brain — on 32 children who had the gene variant. Half had an autism spectrum disorder, while half were developing normally.

Regardless of whether the kids had autism or not, children with the CNTNAP2 “risk” gene showed more activity in the frontal lobe of the brain (specifically, inside the prefrontal cortex) during a “language learning” task than those without the risk gene.

In those without the risk gene, activity in the prefrontal cortex decreased during the task. Instead, there was more connectivity between the frontal lobe and other areas of the brain, including the left side of the brain, which is involved with language.

In children with the risk gene variant, brain scans showed that the frontal lobe was “over-connected” to itself rather than connected in a normal fashion with the rest of the brain.

“In the kids who carry the risk version, instead of having nice clean connections between frontal lobe and the left side of the brain where you process language, you see more of a broad connection throughout the frontal lobe, almost as if it’s talking to itself,” said lead study author Ashley Scott-Van Zeeland, a postdoctoral research fellow at Scripps Translational Science Institute in San Diego.

“Instead of having a highly efficient connection to the left side of the brain, they are sampling all over the frontal lobe,” the researcher said. “That could help explain some of the language difficulties in autism.”

Scott-Van Zeeland did the research as a doctoral candidate at University of California, Los Angeles.

The CNTNAP2 gene helps “wire” the brain: It is known to be active in utero when the structure of the brain is forming, Scott-Van Zeeland said. Unlocking its role in brain function could help in the development of new early interventions or treatments for autism.

Experts point out, however, that the gene variant is part of the spectrum of normal gene variation, and those who have it will not necessarily develop any intellectual disorder.

With some one-third of the population possessing the gene variant, not everyone who has the variant has autism, of course. Conversely, there are plenty of people with autism who don’t have the variant, said Dr. Gary Goldstein, president and CEO of the Kennedy Krieger Institute in Baltimore, which serves children and teens with autism and other developmental disorders. Goldstein was not involved with the study.

So it’s important to keep in mind that while people who have the CNTNAP2 variant may be more susceptible to autism, there are other genetic or environmental triggers involved, he explained.

But this study does “break new ground,” he said, in correlating a known risk gene for autism with alterations in brain activity, as seen in functional MRI imaging.

Previous studies have linked certain genes with autism, while others have shown an association between brain imaging and a child’s behavior. But this is among the first to show how a gene associated with autism might alter how the brain functions, Goldstein said.

Think of the brain like an orchestra, he suggested. When the piano starts to play, the rest of the orchestra turns down the volume in order to hear the piano. In autism, perhaps the volume isn’t being “turned down” inside the frontal lobe, making it difficult to shift attention to other things, he said.

“What they are really showing is when you have this risk gene, this part of the brain doesn’t tune down in the same way it does in people who don’t have it,” Goldstein said. “That’s consistent with what’s seen in kids with autism. They have these very focused interests. They may very good at what they are doing, but they’re not engaging others or the rest of their brain.”

WEDNESDAY, Nov. 3 (HealthDay News) — When Medicare cut its reimbursement rates for hormonal treatment for men with prostate cancer, use declined drastically.

But patients who really stood to benefit from the treatment — androgen-deprivation therapy — still received it, researchers report in the Nov. 4 issue of the New England Journal of Medicine.

“Probably what we’re witnessing here is how physicians potentially respond to financial incentives and issues of medical uncertainty,” said study author Dr. Vahakn Bedig Shahinian, an assistant professor of internal medicine at the University of Michigan, in Ann Arbor. “That probably cuts across a variety of medical scenarios having nothing to do with prostate cancer.”

Other experts agreed.

“Clearly, there are other areas where this would occur, even in other areas of prostate cancer right now. For instance, proton beam radiation therapy reimburses very well so there are certain medical centers that are really pushing it,” agreed Dr. Judd W. Moul, chief of the division of urologic surgery at Duke University Medical Center in Durham, N.C.

“Another classic example is imaging,” Moul said. “CAT scans reimburse pretty well so a lot of doctors put CAT scan units into their offices. That’s a profit center.”

The Medicare Modernization Act that went into effect in 2004 and 2005 slashed physicians’ reimbursement for androgen-deprivation therapy by about 50 percent.

In the 1990s, when reimbursement rates were much higher, the number of men receiving the therapy exploded. The treatment is considered effective in more advanced, higher-risk cancers and not so useful in those with low-grade tumors still confined to the prostate, although new evidence on hormonal therapy continues to emerge.

“There was a significant difference between what the doctors were able to buy the drugs for and what Medicare would cover when they were administered,” said Moul. “The bottom line is they were very profitable for the physicians.”

To assess the impact of the Modernization Act, Shahinian and his colleagues looked at records of almost 55,000 men diagnosed with prostate cancer between 2003 and 2005. Patients were separated out based on the characteristics of their tumors — for example, localized vs. locally advanced, and whether evidence existed to support use of this therapy in those cases. An intermediate category was established for cases where the decision was less clear-cut.

Unnecessary use of androgen-deprivation therapy declined from 38.7 percent in 2003 to 30.6 percent in 2004, then to 25.7 percent in 2005, the study authors reported.

But there were no changes in appropriate use of the therapy, which stayed stable, the researchers said. Meanwhile, gray-zone use declined in 2005, but not the prior year.

Research identifying some adverse effects of hormonal therapy may also have contributed to the reduction, the authors said.

Whether these findings — or whether similar reimbursement changes in other areas — will affect the U.S. health-care system isn’t clear, especially with the current push towards personalized medicine.

“It’s very subjective. There are no hard-core guidelines. There are suggestions,” Moul said. “In general, you want to use hormone therapy in higher-risk men who have worse disease but there’s a fairly significant gray zone,” he pointed out.

“There’s probably many, many areas where the system could be tightened up, where we could save money but the downside is going to be a lot of individual patients who are going to be mad because they can’t get what they want to get,” he added.

Shahinian is a consultant for the biotechnology firm Amgen. The American Cancer Society funded this study.

Good Conversation Can Boost Brain Power, Study Finds

WEDNESDAY, Nov. 3 (HealthDay News) — Friendly discussions with other people can help you solve common life challenges, but conversations that are competitive in tone aren’t helpful, finds a new study.

“This study shows that simply talking to other people, the way you do when you’re making friends, can provide mental benefits,” lead author Oscar Ybarra, a psychologist and researcher at the Institute for Social Research at the University of Michigan, said in a university news release.

In this study of 192 undergraduates, the researchers examined the effect that brief episodes of social contact had on a type of cognition called executive function, which includes working memory, self-monitoring, and the ability to suppress external and internal distractions. These mental processes are essential in dealing with day-to-day problems.

Engaging in a short, 10-minute conversation in which they got to know another person helped boost the participants’ performance on a variety of cognitive tasks. But when the conversations had a competitive tone, the participants showed no improvement on the cognitive tasks.

The study will be published in an upcoming issue of the journal Social Psychological and Personality Science.

“We believe that performance boosts come about because some social interactions induce people to try to read others’ minds and take their perspectives on things,” Ybarra said. “And we also find that when we structure even competitive interactions to have an element of taking the other person’s perspective, or trying to put yourself in the other person’s shoes, there is a boost in executive functioning as a result.”

The findings suggest that having a friendly talk with a colleague before a big test or presentation may prove beneficial, according to the researchers.

Hemophilia Drug Used Off-Label Raises Clot Risk

WEDNESDAY, Nov. 3 (HealthDay News) — A drug approved to help staunch bleeding in people with hemophilia raises the risk of heart attacks and stroke when it’s used to stop life-threatening bleeding due to other conditions, such as trauma or surgery, a new study finds.

Despite the nearly twofold rise in risk of dangerous clots that form inside blood vessels and cause heart attacks and stroke in those without hemophilia, researchers said doctors should use the information to carefully weigh the risks and benefits of using the drug, called recombinant activated factor VII (rFVIIa), for any “off-label” use.

“If you are a physician confronted with a patient with excessive blood loss, and you have done everything you can and this patient is almost dying because of the bleeding, it’s quite acceptable to give the drug even if it’s associated with a risk of thrombosis [clotting],” said study author Dr. Marcel Levi, a professor of Medicine at University of Amsterdam in the Netherlands. “On the other hand, if you are confronted with a patient who has less than excessive blood loss and you can try other things, then the twofold increased risk of thrombosis is considerable and I would not give the drug.”

The study is published in the Nov. 4 issue of the New England Journal of Medicine.

What makes the study so unusual is that the drug maker, Novo Nordisk, gave researchers access to unpublished data on the drug and its potential side effects, said Dr. Louis Aledort, a professor of medicine at Mount Sinai School of Medicine in New York City, who wrote an accompanying editorial.

“The company is to be congratulated,” Aledort said. “It would be really great if other companies would do the same so that clinicians thinking about using it a drug in an off-label setting would have all the information at hand so they could make the right decision, which is benefit vs. risk.”

rFVIIa was approved about 10 years ago by the U.S. Food and Drug Administration for use in people with hemophilia. Hemophilia is a group of blood disorders in which the blood doesn’t clot properly, raising the risk of prolonged bleeding, internal bleeding and bleeding into the joints.

Increasingly, the drug has also been used for so-called “off-label” use, or to treat conditions not expressly licensed by the FDA, Aledort said. While it is legal for physicians to use a drug in this way, a drug’s maker cannot promote its use for anything but the condition for which it is approved.

Such “off-label” uses include stopping brain bleeds and staunching bleeding during surgery due to trauma or advanced liver disease, according to the study.

Despite it’s effectiveness in stopping bleeding and promoting beneficial clotting, there have been reports of complications, including heart attacks and stroke.

In the study, researchers looked at 35 published and unpublished studies on off-label usage of rFVIIa to treat or prevent bleeding. The studies involved more than 4,100 people who experienced excessive bleeding, some of whom were given rFVIIa and some who weren’t. Overall, about 11 percent experienced arterial thrombosis, most often a heart attack, unstable angina (a precursor to a heart attack) or stroke.

About 5.5 percent of those who received rFVIIa experienced arterial thrombosis, compared to 3.2 percent of those who were given a placebo.

About 2.9 percent of those who received rFVIIa had a heart attack compared to about 1.1 percent given a placebo.

The risk of dangerous clots rose with age. Among those 65 and older, 9 percent of those given rFVIIa experienced arterial thrombosis compared to 3.8 percent given a placebo. For those 75 and up, rates rose to 10.8 percent and 4.1 percent, respectively.

There was no increased risk of venous thrombosis, such as clots in the legs or lungs (pulmonary embolism).

“The company helped us. They opened their files. They showed us unpublished data on complications. It’s exemplary that the company did this,” Levi said. “Usually companies keep that information a little bit obscure.”

Aledort said this type of research should serve as a template for the rest of the pharmaceutical industry to follow. “It was very brave of the company,” he said.
